In a preliminary design concept, Poison Mushroom Cookie had more of a skull motif, especially in regards to their head their face and cap were placed in such a way that it resembled a human skull, with their mouth taking a W-shaped 'lumpy' form to resemble a stylized alveolar process and top row of teeth. They have a 'lumpy' zigzag-shaped mouth and purple, sunken eyes. They also have what appears to be a giant purple mushroom cap on their head with fuchsia-colored spots.
